- Release of Information/Request Records: If you would like a copy of your records or report sent to another provider, entity or yourself, please complete all sections and return to our office.
Please contact our office via phone to inquire about services, current waitlists status or to confirm the receipt of your materials. If we are accepting clients, we will schedule a phone screener and/or add you to the appropriate waitlist. When an opening becomes available, we will call to set up an intake appointment. Paperwork can be returned via the methods listed below and is required before your fist appointment.
Waitlists for Adult ADHD/LD and Autism Assessments are currently closed. Check back late August 2024 for updates.
As a public institution, Washington State University is obligated to produce, upon request, any records or documents it keeps of its activities. Although identifying information would be removed, all email is subject to public records requests. More information can be found here on the university website.
