Once the completed paperwork is received by our office, we will add you on our waitlist*. It’s also strongly recommended that you call to confirm the receipt of your information. When we have an opening, we will call to set up your intake appointment.  Paperwork can be returned via the methods listed below.

*Waitlists for Adult Therapy, Adult ADHD/LD and Adult Autism Assessments are currently closed and we are not accepting new paperwork for those services at this time. Availability will be evaluated in early Spring 2023.
